HOPWA: Housing Opportunities for People with Aides

Full Time Days

Lutheran Social Services of Central Ohio is currently seeking a HOPWA C ase Manager for Faith Mission in Columbus Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio , providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as an HOPWA Case Manager with Lutheran Social Services?

  • Work with HOPWA Program Manager and, when applicable, shelter supportive services staff to identify housing for clients. As needed assist the HOPWA Program Manager with inspecting housing for occupancy for clients enrolled to program from waitlist/referrals.
  • Develop formal and informal relationships with various landlords/housing providers in the greater Columbus area. Maintain systems and methodologies consistent with a "housing first" philosophy.
  • Work closely with clients to develop and implement intentional action-oriented goal plans based with a focus on maintaining housing and creating a meaningful quality of life.
  • Work with clients to develop action-oriented goal plans with a focus on securing housing, operating from a "housing first" model.
  • Refer clients to partner agencies and community resources that benefit their short-and long-term goals including physical and behavioral health care needs.
  • Works closely with physicians and other healthcare providers to help assure appropriate care and management of patients infected with HIV/AIDS; obtains medical history as appropriate and assists with obtaining appropriate tests and treatment for patients when needed.
  • Assist clients in enrolling and/or maintaining enrollment with appropriate Ryan White Case Management
  • Gather information on potential resources for clients in the larger community.
  • Maintain clear and precise data and case notes and careful documentation of all client interactions.
  • Actively participate in weekly Case Review, working with colleagues toward positive client outcomes.
  • Participates in local or state public health meetings or other local partner meetings as appropriate.
  • Contribute to an atmosphere of safety, sanctuary, dignity, support, and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements
  • Assist with crisis intervention and de-escalation by modeling professional and calm problem-solving behavior for clients, colleagues, and the community.

Requirements for an HOPWA Case Manager with Lutheran Social Services:

  • Four-year degree in Social Work, related field or equivalent experience required.
  • LSW, preferred.
  • Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required.
  • Minimum two years working in a social service setting required.
  • Experience working with the following or related populations: homelessness, behavioral health, substance abuse/addiction, chronic physical disabilities, HIV/AIDS including prevention & education strongly preferred.
  • Experience with HIV/AIDS prevention & education strongly preferred.
  • Ideal candidates will be able to demonstrate experience utilizing (minimally) the following techniques, interventions and EBP's: Motivational Interviewing skills, Stages of Change, Harm Reduction. prevention & education strongly preferred.
  • Strong oral and written skills. Skill in writing case notes, client action plans,
  • Experience working with housing programming strongly preferred.
