Looking for a Part-time or Full-Time Social Worker to join our team!

The Kidney Care Center , located in Hobbs, NM , is a 16- chair hemodialysis center. We also offer Peritoneal Dialysis for patients.

We are seeking a highly motivated and dedicated social worker to join our dialysis team in New Mexico. The ideal candidate will have experience in providing social work services to patients with end-stage renal disease and will possess excellent communication and interpersonal skills.

How you will change lives

As a social worker at The Kidney Care Center, you will be an integral part of a cross- functional

team, working with healthcare providers to facilitate quality care for patients living with kidney disease.

What you will be doing

• Provide emotional support and counseling to patients and their families
• Assess patient and family psychosocial needs and develop care plans
• Coordinate patient care with other healthcare professionals
• Advocate for patients and their families in accessing community resources
• Participate in interdisciplinary team meetings and patient care conferences
• Maintain accurate and up-to-date patient records

Qualifications/Skills:

• Master's degree in Social Work from an accredited institution
• Current state licensure as a Social Worker required
• Experience in providing social work services to patients with end-stage renal disease preferred
• Knowledge of Medicare and Medicaid regulations and policies related to dialysis
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Excellent organizational skills
• Bilingual- Spanish speaking preferred
